About Create form fields

Create form fields turns a flat PDF, such as an application form someone typed up, into one people can fill in on screen. Draw text boxes, checkboxes, drop-down menus and lists where the answers go, and give each a name.

Use it to make a PDF fillable, add form fields to a PDF, create a fillable form, or add checkboxes and drop-down lists.

How to use it

  1. Choose the PDF.
  2. Pick a Field type (Text, Text (several lines), Checkbox, Drop-down or List) and draw it on the page.
  3. Give it a Field name and, for menus and lists, the Options, separated by commas.
  4. Press Add fields and download the fillable PDF.

Good to know

  • Radio buttons, push buttons and signature fields are not offered yet.
  • Fields use Helvetica, which covers Latin text.
  • Give each field a clear, unique name; that is what appears when the answers are exported.

Questions and answers

How do I make a PDF fillable?

Open it here, draw a field over each blank and download. The fields work in Adobe Reader, browsers and most PDF apps.

Can I add drop-down menus?

Yes. Choose Drop-down and list the options, separated by commas.

Can people sign in the form?

Signature fields are not offered yet. People can add a visual signature with the PDF editor.

Will the form work on phones?

Yes, in any PDF app that supports forms.
